Prevention Tool Kit
 

This tool kit is one of a series originally developed under the project, Enhancing Geriatric Care through Practicing Physician Education, funded in 1998 by the John A. Hartford Foundation and administered by the American Geriatrics Society.  Since 2002, the Practicing Physician Education in Geriatrics (PPE) project has continued with the generous support of the Merck Institute of Aging and Health. The tool kits are intended to help physicians and other healthcare providers better understand  'Geriatric Syndromes' and other chronic conditions common in older adults.  Each tool kit contains evidence-based guidelines and references; forms and tools for evaluation, diagnosis and treatment; and educational materials and resources for the public.

The Prevention Tool Kit offers one practical approach to organizing clinical preventive services for older adults in your office practice. This tool kit is based on the Pocket Guide to Staying Healthy at 50+, published by the Agency for Healthcare Research and Quality (AHRQ). The core of this tool kit is the "Staying Healthy for the Older Adult" questionnaire, a modified version of the AHRQ document. This document can help you and your office staff determine your older patients' preferences and informational needs on preventive strategies. This tool kit also contains additional patient education materials to provide an efficient way of handling the questions that arise from the many preventive choices.
